Insurance Options

Our agency provides nursing, physical therapy, speech therapy, occupational therapy, and home health aide services to patients in their homes. These services can only be through your doctor’s orders and are paid for by your insurance carrier as long as you meet the appropriate eligibility guidelines. A brief description of these guidelines is as follows: (Please note, these guidelines do not guarantee payment for services, and for detailed information you should review your individual policy).

Medicare

Medicare will only pay for your services as long as:

1) you are homebound, (that means that it must take a taxing and considerable effort on your part to leave your home).

2) skilled services are medically necessary on an intermittent or part-time basis:

3) you remain under a physician’s care while receiving services.

Medicaid

Home Health Services are Medicaid State Plan services that are provided on a part-time and intermittent basis to Medicaid consumers of any age. Home health services include home health nursing, home health aide, and skilled therapies (physical therapy, occupational therapy, and speech language pathology). The only provider of home health services is a Medicare Certified Home Health Agency (MCHHA).

The medical necessity for home health services must be certified be the consumer’s qualifying treating physician. A face-to-face encounter with the consumer and the physician, advanced practice nurse in collaboration with the physician, or a physician assistant under the supervision of the physician is required for certification of medical necessity. A face-to-face encounter must be conducted within the 90 days prior to the home health care start-of-care date, or within 30 days following the start-of-care date, inclusive of the start-of-care date.

Private Insurance

Eligibility guidelines depend on the carrier and individual’s policy. Pre-authorization is usually required and is completed by our office staff.  We accept most private insurances including Medical Mutual.  We are also proud to be CHAPS Accredited.

It is our goal to assist you back to recovery and a normal independent way of life as soon as possible. As you recover, the services of our staff will decrease.
